Responsibilities:
Provide direct patient care under the supervision of licensed nurses, including assistance with activities of daily living (ADLs), vital signs, and mobility
Develop and maintain strong relationships with patients, families, and healthcare team members to ensure seamless care coordination
Monitor and report patient progress to the healthcare team, identifying any changes or concerns
Maintain accurate and timely records of patient care and services
Participate in patient care conferences and interdisciplinary team meetings to discuss patient plans and goals
Collaborate with other healthcare professionals to develop and implement individualized care plans
