Position Summary

The LPN Case Manager plays a vital role in coordinating and overseeing care for individuals with physical and developmental disabilities in home and community-based settings. This position is responsible for completing initial individual assessments, developing individualized plans of care, and ensuring that ongoing services meet quality standards. The LPN Case Manager works collaboratively with caregivers, individuals, families, and interdisciplinary teams to promote optimal health outcomes and individual satisfaction.

Key Responsibilities

1. Care Coordination and Case Management

Assess the physical, emotional, and developmental needs of individuals with physical and intellectual disabilities in their home environment.

Develop individualized care plans based on assessments, collaborating with Case Managers, physicians, therapists, and other healthcare providers as needed.

Oversee personal care and companion service delivery in home and community-based environments, ensuring compassionate, person-centered care.

Serve as the primary point of contact for individuals and their families, ensuring clear communication and coordination of services.

Monitor individuals’ progress and make adjustments to care plans as needed, based on responses and evolving needs.

Coordinate schedules for new and existing individuals to ensure staffing meets the appropriate level of care and desired outcomes.

2. Referrals, Staffing Coordination, and Onboarding

Referral Generation: Drive business by generating referrals from community partners for individuals with physical and developmental disabilities in need of services and supports.

Assess Staffing Needs: Work with the RN Regional Manager to assess staffing requirements for current and upcoming referral needs, ensuring the right balance of skills and expertise.

Referral Process Management: Oversee the referral process, ensuring that staffing referrals are timely, accurate, and aligned with individual needs and standards.

Onboarding Support: Coordinate the onboarding process for candidates with the recruitment team, ensuring that all necessary documents, background checks, and certifications are completed promptly.

3. Collaboration and Communication

Communicate with the individual’s interdisciplinary care team, including Case Managers, physicians, therapists, social workers, and caregivers, to ensure comprehensive, coordinated care.

Educate individuals and families about care requirements and strategies for managing health at home.

Develop and implement strategies to address any concerns or barriers to effective care, ensuring individual and family satisfaction.

4. Compliance and Documentation

Monitor and oversee field-based CNAs and in-home caregivers, ensuring that daily nursing notes align with individual needs and outcomes.

Provide training and guidance to field staff, family members, and team members on best practices in care delivery, person-centered planning, and internal policies.

Accurately document and maintain all required records, including service plans, assessments, service notes, and incident reports.

Ensure that any incidents involving individuals are documented and reported according to regulatory requirements.

5. Health Education and Prevention

Provide education to individuals and families on managing health conditions and preventive health measures as needed.

Offer guidance on healthy lifestyle choices, symptom management, and strategies for reducing the risk of complications.

Educate families on strategies for improving quality of life and promoting independence.

6. Quality Assurance and Safety

Ensure that all care is delivered in accordance with best practices, safety protocols, and quality standards.

Participate in regular case reviews and quality assurance initiatives to improve care delivery.

Advocate for improvements in individual care and identify opportunities to enhance care quality and service delivery.

Qualifications and Candidate Profile

Licensure

Valid Licensed Practical Nurse (LPN) license in South Carolina.

Experience

At least 2 years of nursing experience, with a focus on home health or working with individuals with IDD preferred.

Experience in case management, care coordination, or interdisciplinary collaboration highly desirable.

Clearance Requirements

Criminal Background Check.

Motor Vehicle Record Check.

Child Abuse Clearance.

Authorization to work in the United States.

Reports To: RN Regional Manager.

Competencies

Ability to develop and implement individualized care plans based on comprehensive assessments.

Knowledge of IDD-related health conditions, treatment options, and care strategies.

Strong communication skills for effective collaboration with individuals, families, and interdisciplinary teams.

Ability to work independently, manage a caseload, and prioritize tasks effectively.

Cultural Competence: Sensitivity to and understanding of diverse backgrounds and individual needs.

Problem-Solving and Decision-Making: Strong analytical abilities and sound judgment based on data and organizational priorities.

Work Environment

Office-based setting, Monday–Friday, 8:30 AM–5:00 PM.

Requires travel to visit individuals in their homes, community settings, or healthcare facilities.
