Key Responsibilities
Healthcare Plan Management
- Implement patient care plans under physician direction
- Coordinate with healthcare personnel to optimize patient outcomes
- Assist with bathing, grooming, dressing, and other daily living activities
- Provide palliative and comfort-focused care to enhance quality of life
- Record temperature, pulse, and blood pressure regularly
- Observe for abnormalities and promptly report findings
- Administer prescribed medications with accuracy
- Maintain comprehensive and compliant medication documentation
- Identify and treat wounds, pressure sores, and infections
- Apply appropriate care techniques and communicate updates to healthcare teams
- Train family or in-home caregivers on patient handling, hygiene, and daily routines
- Provide continuous support and instruction to ensure safe and effective care
- Experience with Peg Tube/G-Tube care
- Proficiency in Trach/Vent management
Education:
- Completion of an accredited LPN program
- Minimum of one year of nursing experience
- Valid In-State or Multi-State LPN license
- CPR/BLS certification
- Current TB test
