🩺 What You’ll Be Doing:
• Deliver comprehensive, patient-centered primary care
• Focus on prevention, chronic care management, and long-term outcomes
• Conduct Annual Health Assessments (AHA) & Chronic Care Management (CCM)
• Collaborate with interdisciplinary teams to coordinate care
• Advocate for patients and improve access across the healthcare system
• Utilize evidence-based medicine and clinical best practices
🎯 What We’re Looking For:
• Board Certified Physician (MD/DO)
• Active California license
• 1–2 years of clinical experience (3 preferred)
• Experience with Epic or similar EHR
• Passion for patient advocacy and team-based care
