Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by observing vital signs, managing catheter care, and recognizing changes in condition related to geriatrics, dementia, Alzheimer's disease, or end-of-life care.
- Administer medication accurately following medical documentation protocols while maintaining compliance with home healthcare regulations and HIPAA confidentiality standards.
- Record detailed medical records and documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure precise patient data management.
- Support patients during transfers and mobility activities utilizing proper patient transfer methods to ensure safety for both the patient and caregiver.
- Provide emotional support and effective communication tailored to individuals with developmental disabilities or cognitive impairments such as dementia or memory care needs.
- Maintain a clean, safe environment by assisting with laundry, cleaning, meal preparation, and ensuring adherence to infection control standards.
Experience
- Previous caregiving experience in assisted living facilities, nursing homes, hospice or home healthcare settings is highly preferred.
- Demonstrated knowledge of medical terminology, medication administration, first aid, basic life support (BLS), and CPR certification is essential.
- Experience working with geriatric patients or individuals with disabilities—including developmental disabilities—is highly valued.
- Familiarity with home health agency operations, hospice care procedures, and home & community care practices enhances your ability to succeed in this role.
- Proven ability to work with EMR/EHR systems for accurate medical documentation and client communication is advantageous.
- Prior experience handling patient transfers using methods like Hoyer lifts or other assistive devices ensures safe mobility assistance.
- Ability to perform heavy lifting when necessary while maintaining proper body mechanics aligns with safety standards for patient care.
