Registered Nurse (RN) Case Manager

One Rheumatology | Palm Beach Gardens, FL

Position Overview

One Rheumatology is seeking an experienced, highly organized RN Case Manager to play a central role in maintaining clinical excellence and expanding patient access. In this role, you will act as an extension of Dr. Clifford Stermer, supporting complex patient care, clinical workflows, and team coordination while upholding the highest standards of judgment, professionalism, and patient safety.

Key Responsibilities

Clinical Leadership & Direct Provider Support

• Maintain exceptional standards of patient care on behalf of Medical Director, Dr. Stermer by overseeing clinical accuracy, workflow efficiency, patient safety, and care continuity.
• Serve as an extension of the physician to support an organized, efficient, and positive clinical environment.
• Manage and triage clinical messages, medication questions, patient concerns, and follow-up communication.
• Anticipate daily clinical needs and proactively address care gaps to support provider efficiency and expanded patient access.
• Ensure continuity of care by monitoring overdue labs, imaging, follow-ups, and disease-monitoring intervals.
• Provide timely, clinically sound communication to patients and ensure all care loops are closed.
• Independently resolve matters within nursing scope while escalating appropriate clinical issues.
• Collaborate with the Clinical Trials and Research team to ensure appropriate clinical access and information sharing.

Infusion Suite Oversight

• Provide clinical oversight of rheumatology infusion services, including biologics, specialty injectables, and immunomodulatory therapies.
• Assist with ordering infusion medications and coordinating treatment plans.
• Verify dosing accuracy, weight-based calculations, safety labs, pre-medications, and infusion readiness.
• Monitor patients during infusions, identify adverse reactions, and respond appropriately.
• Ensure compliance with infusion protocols, safety standards, and documentation requirements.
• Assume primary operational responsibility for the infusion suite every Thursday and Friday beginning February (following training).
• Partner with billing and authorization teams to ensure infusions are scheduled appropriately and supported by required clinical documentation.

VIP & Concierge Patient Support

• Support concierge and VIP patient messaging with timely, high-touch communication.
• Maintain continuity across VIP care plans, follow-ups, refills, and clinical escalations.
• Ensure white-glove service while maintaining clinical rigor and documentation standards.

Team Collaboration & Leadership

• Partner with the Medical Assistant team to ensure efficient workflows, accurate documentation, and appropriate triage escalation.
• Maintain high standards for chart preparation, rooming, patient flow, and clinical operations.
• Serve as a collaborative and supportive clinical leader who guides, mentors, and uplifts the team.

Qualifications

• Active Registered Nurse (RN) license in Florida (required)
• Strong clinical judgment and experience in complex or specialty care preferred
• Excellent organization, communication, and clinical decision-making skills
• Ability to work independently while collaborating closely with physicians and staff
• Infusion therapy experience strongly preferred (biologics, specialty meds, oncology, rheumatology, GI, or similar)
• Strong knowledge of infusion safety, monitoring, and adverse reaction management
