Overview
The RN Case Manager provides comprehensive care coordination and support to patients across multiple care settings. This role is responsible for developing, implementing, and evaluating individualized care plans, coordinating discharge planning, and managing complex outpatient needs to ensure continuity and quality of care.
The RN conducts prospective, concurrent, and retrospective reviews of clinical information to determine medical necessity, appropriateness of services, and cost-effectiveness. Acting as a patient and family advocate, the Case Manager ensures that all care decisions align with clinical standards and patient goals. In some cases, the RN may perform in-home or hospital evaluations to assess patient needs and facilitate care transitions.
Qualifications:
· Associate Degree or higher in Nursing (required)
· Active and unrestricted Registered Nurse (RN) license (required)
· Certification in Case Management (required or obtainable within one year of hire)
· Minimum of two (2) years of experience in medical case management (required)
· Current Basic Life Support (BLS) certification (required)
· Valid driver’s license and ability to travel as needed
· Broad knowledge of healthcare systems and current standards of clinical practice
· Strong organizational, communication, and interpersonal skills
· Proficiency with Microsoft Office and electronic medical record systems
Flexibility to work evenings, weekends, and holidays as needed
Pay: $82,450.18 - $95,000.00 per year
