One Home Health is seeking an experienced home health nurse to support our Indiana team in a part-time capacity. Openings are available in Lake County, South Bend, and the surrounding areas.
What You'll Do
- Conduct ongoing observation, assessment, and documentation of patient symptoms and progress.
- Perform initial home care visits and reassess patient needs and progress as required.
- Collaborate effectively with interdisciplinary team members to ensure coordinated care.
- Demonstrate proficiency in accurate and timely OASIS documentation.
- Provide supervision and guidance to Home Health Aide (HHA) field staff as appropriate.
- Adhere to all applicable Medicare regulations and compliance standards.
- Promptly notify the physician of any changes in the patient’s condition.
- Coordinate care with other disciplines to support comprehensive patient needs.
- Complete all documentation and paperwork in a timely and efficient manner.
- Educate patients and caregivers on disease processes, medications, care plans, and treatments in accordance with policies and procedures.
- Deliver high-quality care to patients in their homes, following established policies and procedures.
- Perform wound care, including assessment and treatment, as well as IV line care and maintenance.
What You Bring
- Graduate of an accredited school of professional nursing; Bachelor of Science in Nursing (BSN) or Public Health Nurse (PHN) degree preferred.
- Current Registered Nurse (RN) license in the applicable state(s) of practice.
- Minimum of one (1) year of experience in an acute care setting or equivalent clinical experience.
- Valid driver’s license with a reliable, well-maintained vehicle; must meet state and/or agency insurance requirements and maintain a good driving record.
- Current CPR certification required.
- Strong communication and interpersonal skills.
