What You'll Do
- Conduct comprehensive in-home occupational therapy evaluations and deliver targeted treatment interventions
- Develop and implement individualized care plans designed to maximize patient independence and functional outcomes
- Educate patients and caregivers on safety practices, adaptive techniques, and proper use of assistive equipment
- Complete accurate, timely, and compliant clinical documentation for all patient visits
- Collaborate effectively with the interdisciplinary team to communicate patient progress and adjust care plans as needed
- Ensure adherence to all clinical, regulatory, and documentation standards
What You Bring
- Active licensure as an Occupational Therapist (OT) in good standing
- Prior home health experience preferred; new candidates with strong clinical skills encouraged to apply
- Excellent communication and clinical documentation abilities
