This patient-facing role is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing.

Type: Full-Time: 40 hrs
Schedule: M-F 8am-5pm/1 hr lunch
Wage: $18.91-$19.84/hr DOE
KEY RESPONSIBILITES:
- Care Coordination:
Facilitate warm hand-offs and referrals between patients, providers, and community services. - Community Representation:
Participate in public outreach events and community consortiums to promote One Health services and build strong local partnerships. - Insurance Assistance:
Support patients in completing applications and navigating enrollment for public and private insurance programs. - Screen and Support:
Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers. - Documentation & Reporting:
Maintain detailed electronic records of patient encounters, referrals, service plans, and outcomes.

- Proficient computer skills, including internet-based applications
- Ability to thrive in a fast-paced environment while managing multiple priorities
- Strong organizational skills and accountability for time and activities with remote supervision
- A passion for serving others with professionalism and clear boundaries
- Community involvement or membership in the community served
Requirements:
- High School Diploma or GED
- Valid Driver's License
