One Health in Sheridan, Wyoming is seeking a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) to join our integrated care team. 🧠 Key Responsibilities
- Evaluate, diagnose, and treat patients within your licensed scope
- Provide therapy, preventive guidance, case management, and specialty referrals
- Consult with primary care and care team members to support comprehensive patient care
- Maintain accurate and timely electronic health records
- Participate in community outreach, education, and integrated care initiatives
📋 Qualifications
Required
- Master’s degree in Social Work, Counseling, or Marriage & Family Therapy
- Licensed in Wyoming as an LCSW, LPC, or LMFT
- Experience with individual therapy and treatment plans
- BLS certification upon hire
- Valid driver’s license
Preferred
- Two or more years of experience in community or behavioral health
- Experience working with diverse populations
