The Tribal Health Navigator oversees the One Community Health (OCH) outreach program providing healthcare services to the Native community and local underserved populations in the Columbia River Gorge. In partnership with the Tribes, the Tribal Health Navigator helps OCH tailor its care delivery approach to local needs. The Tribal Health Navigator acts as a bridge between OCH and the Native communities in the Columbia River Gorge, building authentic, long-term relationships and improving healthcare access and outcomes, and striving to eliminate health disparities. This position will support our patient's needs, specifically helping Natives navigate the healthcare system and access care when they need it. The Tribal Health Navigator also provides health and safety group education and training for community-identified needs and connects Tribal members with community partners and additional local resources. This position also supports broader One Community Health awareness and outreach efforts as needed.

This position is full-time, Monday-Friday with some occasional evenings and weekends. The primary work site is located at our clinic in Hood River, Oregon with travel to various outreach locations via company vehicle. One Community Health support training for a Community Health Worker certification for this role.

Minimum Qualifications

  • High School Diploma or equivalent
  • Minimum two years of experience working directly with Tribal communities and/or Native American community-based organizations.
  • Minimum of two years of experience in a health-related, community health, public health, or social service field.
  • Strong interpersonal and communication skills with the ability to work respectfully and effectively with patients, families, Tribal leaders, community partners, and interdisciplinary clinical teams.
  • Commitment to providing culturally and linguistically appropriate, patient- and family-centered care coordination.
  • Able to maintain patient privacy and compliance with healthcare regulations such as HIPAA.
  • Ability to prioritize, organize, and manage work across multiple and competing timelines.
  • Demonstrated initiative, sound judgment, and problem-solving skills in complex or evolving situations.
  • Proficiency with Microsoft 365, including OneDrive, Excel, Outlook, and PowerPoint.
  • CPR/BLS certification, or ability to obtain within six months of hire with One Community Health support.
  • Ability to travel frequently within the Columbia River Gorge.
  • Valid driver’s license, clean driving and insurance record, minimum of 2 years driving experience
  • Ability to meet organizational driving and background-check requirements, including operation of the Wy-Kan-Ush Pum Mobile Unit.

Preferred Qualifications

  • Bachelor’s in public health or community health, or clinical experience/training (e.g., medical assistant)
  • Experience managing a program serving the health needs of Indigenous or Tribal communities and coordinating care with the Indian Health Service and Tribal health partners.
  • Experience using electronic health record systems (e.g., Epic).
  • Familiarity with traditional healing practices and Indigenous approaches to wellness.
  • Experience providing group education, training, or facilitation related to health, safety, prevention, or wellness.
  • Community Health Worker or Traditional Health Worker Certification, or ability to obtain within 6 months of hire with One Community Health support.
  • Certified Assister for Washington Apple Health and/or Oregon Health Plan, or ability to obtain within 6 months of hire with One Community Health support.

Compensation

  • Salaried/Exempt: $53,040–$56,160 annually, based on experience and certification(s).
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
