The Case Manager assesses plans, implements, coordinates, monitors and evaluates options and services to meet individual healthcare needs. Promotes quality and cost-effective outcomes.
Responsibilities
- Interject objectivity, maximize efficiency and work collaboratively with patient, physician, family and in the development and execution of a healthcare plan.
- Assist client’s self-direct care and make decisions to the degree possible.
- Appraise the need for intervention by gathering relevant data and conducting an objective evaluation.
- Evaluate triggers to identify potential clients suitable for effective case management intervention.
- Promote effective communications between all team members, client, family, provider and other parties involved in the case.
- Ensure the clients best interests are represented in the evaluation, treatment and management of their health care needs.
- Develop a healthcare plan that addresses the immediate, short-term and ongoing needs as well as where and how client’s care needs to be met.
- Routinely make comprehensive and independent assessment of the client’s status and progress toward reaching the goals set forth in the care plan.
- Performs other duties as assigned.
Education:
- Bachelor's Degree in Nursing (BSN) is required.
- Minimum two (2) years of experience in a health related setting or Community Based Organization.
- Excellent relationship building skills.
- Excellent and effective written/verbal communication skills.
- Ability to effect change.
- Critical analysis.
- Excellent planning and organizing skills.
- Ability to promote client/family autonomy.
- Knowledge of funding resources, services, and clinical standards and outcomes.
- Ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position.
- BLS Certification is required.
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
