Job Summary:
We are seeking a compassionate and experienced Skilled Nurse (RN) to provide high-quality clinical care to patients in a Skilled Nursing Facility or home-health environment. The ideal candidate will deliver skilled interventions, monitor patient conditions, maintain accurate clinical documentation, and collaborate with the interdisciplinary care team to ensure optimal patient outcomes.
Key Responsibilities:
- Provide skilled nursing care including assessments, medication administration, wound care, IV therapy, catheter care, and pain management.
- Complete comprehensive patient assessments and document findings in compliance with state and federal guidelines.
- Monitor and evaluate patient progress, reporting changes to the physician and care team.
- Develop, implement, and update patient care plans based on individual needs.
- Educate patients and families on medications, disease processes, after-care instructions, and safety measures.
- Maintain accurate and timely clinical documentation (SN notes, care plans, visit logs).
- Collaborate with PT, OT, ST, social workers, and other healthcare professionals for coordinated care.
- Adhere to all HIPAA, OSHA, Medicare, and state regulations.
- Ensure a safe and supportive environment for all patients.
Qualifications:
- Active Massachusetts RN License (required).
- Minimum 1 year of nursing experience (SNF or home-health experience preferred).
- Strong clinical assessment and critical-thinking skills.
- Ability to manage time effectively and work independently.
- Excellent communication and patient-care skills.
- Familiarity with EMR systems, Medicare guidelines, and skilled-nursing protocols.
