LCSW or LICSW is strongly preferred, but we will consider experienced non-licensed candidates with strong SNF experience.

Compensation ranges from $50,000–$130,000 based on licensure, SNF experience, and responsibilities. Experienced licensed candidates may have the opportunity to support two facilities, with compensation reflecting the expanded role.

- Support residents and families throughout their stay
- Complete psychosocial assessments and required documentation
- Participate in care planning and interdisciplinary meetings
- Coordinate discharge planning and community resources
- Address resident and family concerns
- Collaborate with nursing, rehabilitation, admissions, and leadership
Requirements
- Skilled nursing (SNF) or long-term care experience required
- Strong communication and organizational skills
- LCSW or LICSW preferred
