Job Description:
We are seeking a compassionate, organized, and motivated Case Manager to join our team in Lynn, MA. This role focuses on providing hands-on support to individuals by assessing their needs, creating effective care plans, and coordinating essential resources that promote stability, independence, and improved quality of life. The ideal candidate is a strong communicator who takes a proactive approach to helping clients achieve their personal and financial goals.
Main Responsibilities:
- Conduct thorough intake assessments and psychosocial evaluations to understand each client’s circumstances, strengths, and challenges.
- Develop and manage personalized care plans that address both short-term needs and long-term objectives.
- Offer support, guidance, and counseling to help clients navigate barriers and make positive life changes.
- Apply motivational interviewing techniques to encourage self-reliance and goal achievement.
- Connect clients with community resources, programs, and services suited to their needs.
- Provide life skills coaching and mentorship to help clients build confidence, stability, and independence.
- Respond to crises professionally, using effective de-escalation and problem-solving strategies.
- Keep accurate and up-to-date records and progress reports, revising care plans as client situations change.
