Position Summary:
The Community Liaison / Home Health Care Marketer is a critical, outcomes-driven sales role responsible for aggressively building OMAMA's book of business and expanding our patient base. This position is the primary engine for referral generation, requiring a skilled, results-focused professional who can establish and nurture lucrative partnerships across the healthcare continuum.
The ideal candidate is an energetic, highly competitive sales professional who views every interaction as an opportunity to secure a new referral source and capture market share for OMAMA Home Health Care.
Key Responsibilities:
1. Aggressive Business Development & Sales Targets:
Establish and solidify high-volume referral relationships with nursing home administrators, discharge planners, case managers, social workers, and physicians to drive continuous patient admissions.
Actively prospect and penetrate new accounts across hospitals, skilled nursing facilities, rehabilitation centers, and physician practices to strategically expand the agency's service area and revenue stream.
Maximize market share by conducting regular, results-focused on-site visits to secure commitment for OMAMA as the preferred home health provider.
Lead the sales cycle from introduction to close, ensuring a consistent pipeline of qualified referrals.
Achieve or exceed all assigned monthly and quarterly referral and admission goals.
2. Strategic Account Management & Market Intelligence:
Promote the value proposition of OMAMA's skilled nursing, therapy, and personal care services with a keen understanding of competitive advantages.
Maintain comprehensive knowledge of Medicare, Medicaid, and Managed Care dynamics to efficiently navigate the referral process and overcome payer obstacles.
Document all sales activities, pipeline status, and referral outcomes to provide accurate forecasting and ROI analysis for leadership.
Provide strategic feedback to leadership on market trends, competitor activity, and new opportunities for revenue generation.
3. Referral & Patient Conversion:
Serve as the primary liaison for high-priority referral sources, ensuring rapid, professional response times to secure the patient admission.
Conduct highly engaging family meetings upon referral to professionally introducing OMAMA's services, clearly articulate the care plan, and facilitate a smooth transition to home care.
Collaborate tightly with the Intake and Clinical teams to ensure seamless handoffs, guaranteeing prompt admission and maintaining high referral source satisfaction.
Qualifications:
Required Experience & Skills:
. Minimum of 2 years of demonstrably successful sales or business development experience within home health care, hospice, or post-acute care.
. Proven track record of meeting or exceeding aggressive sales targets.
. Existing relationships and a strong network across local hospitals, skilled nursing facilities, and physician practices are highly valued.
. Strong closing skills and a results-oriented approach to sales.
. Valid driver’s license and reliable transportation required for frequent travel throughout the assigned territory.
