About Us Olive Tree Haven is a newly licensed Congregate Living Health Facility (CLHF) in Colton, CA, providing 24-hour skilled nursing care in a warm, homelike setting. We are building our founding care team and looking for compassionate, skilled professionals who want to be part of something meaningful from the ground up.
Position Summary We are seeking a Licensed Vocational Nurse (LVN) to deliver direct nursing care to residents in our intimate, person-centered facility. As part of a small, dedicated team, you will play a hands-on role in day-to-day clinical operations and have the opportunity to shape the care culture from day one.
Responsibilities
- Administer medications and treatments as prescribed by physicians and in accordance with the plan of care
- Monitor and document resident conditions, vital signs, and changes in health status
- Communicate with physicians, families, and interdisciplinary team members regarding resident care needs
- Assist the Director of Nursing in maintaining accurate and timely clinical documentation
- Support residents with activities of daily living as needed
- Follow infection control protocols and ensure a safe, clean environment
- Participate in care planning and quality improvement initiatives
- Respond promptly to resident needs and emergencies
Qualifications
- Active, unrestricted California LVN license (required)
- Current BLS/CPR certification
- Experience in long-term care, skilled nursing, or subacute care preferred
- Strong clinical assessment and documentation skills
- Ability to work independently and collaboratively in a small-team environment
- Compassionate, dependable, and detail-oriented
- Must pass a background check and health screening
