We are looking for an Home Health RN to join our home health team. The RN coordinates the patient’s home care services and follows physician orders in accordance to agency, state and federal guidelines. Provides direct care as needed and/or serve as the direct supervisor for LPN and home health aide team members.
Professional Responsibilities of the Home Health RN
- Provides nursing visits within patient homes including in Assisted Living and Independent Living facilities
- Performs patient assessments based on agency standards of practice to ensure effective and appropriate home care services
- Directs, plans, and initiates appropriate action independently and responsively in home care situations
- Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Aides to provide effective and quality home care services
- Performs job duties in accordance with agency policies, procedures, and professional and community standards
Home Health RN Requirements:
- RN License in good standing
- Valid Driver’s License with vehicle and car insurance
- Must be able to pass a background check
- Willingness to share on call rotation basis (normally 1 weekend a month)
- Excellent verbal and written communication skills
- Able to multi-task efficiently
- Great Attitude & Attendance
- Accountable & Compassionate
