Job Overview
The Ohio Department of Medicaid (ODM) is seeking a healthcare professional with care coordination experience to be part of our agency. As a OhioRISE Care Coordination Manager, your key responsibilities will include:
- Overseeing and monitoring care coordination activities delivered by the OhioRISE plan and contracted care management entities (CMEs).
- Ensuring OhioRISE plan and CME care coordination practices reflect the vision and goals of the OhioRISE Program.
- Assisting in the development of innovative program initiatives designed to improve service coordination and multi-system collaboration for youth and families with significant behavioral health services needs.
- Providing technical assistance to OhioRISE members and community stakeholders to ensure access and engagement with care coordination services.
- Working collaboratively with internal and external stakeholders across a variety of departments, levels, state agencies, and managed care plans to improve care coordination for OhioRISE members.
- Or 12 months experience has Medicaid Health Systems Specialist, 65293.
- Or equivalent of Minimum Class Qualifications for Employment.
