Odd Fellow Nursing Home is looking for a Director of Social Services to join their team!

Role and Responsibilities
A Social Worker is a qualified person who directs Social Services at The Odd Fellow Home. As a member of the health care team, the Social Services Director will work with the health care team to assist in developing a total treatment plan of care for residents. The Social Worker also analyzes the social needs of residents and acts as an advocate for the residents in a rehabilitation and long-term care facility. Setting up services prior to the resident re-entering the community is part of the Social Services responsibility.

Essential Duties and Responsibilities:

  • Has a working knowledge of the basic theory and principles of Social Work practice. Will keep current of new trends and developments within the field and have the capacity to apply these to practice situations.
  • Has knowledge of State and Federal programs and regulations affecting nursing homes, extended care and related facilities.
  • Organize and implement a social Service program within a nursing home, extended care, or related facility.
  • Assist Community Liaison in attaining and/or maintaining the targeted occupancy level.
  • Prepares a comprehensive assessment of each resident.
  • Compile social service Plan of Care, update quarterly and as needed, including discharge plans if/when appropriate.
  • Attends and participates in Care Conferences, IDT and Medicare meetings.
  • Function as a referral source with a working knowledge of community resources.
  • Assists with providing referrals for hospice and post admissions
  • Assists in providing a caring and concerned atmosphere for residents and family desiring admission to the nursing home.
  • Informs residents of Advanced Directives, Residents’ Bill of Rights, bed-hold information, and assures completion of Admission packet as directed by policy and procedure.
  • Provides assistance with resident and family’s adjustment to facility, and continuously assist and communicate with the resident, guardian, family, or other responsible persons.
  • Assists in investigation of resident complaints, self-reporting, and lost items.
  • Participates as a member of the QAPI Committee and provides summary report of resident complaints and trends.
  • Refers residents to community resources upon discharge; maintains community resource file.
  • Devises and maintains a social service policy and procedure manual.
  • Works with the nursing staff in providing consultative services in regards to residents with behaviors and/or who are combative or resistive, and assist with developing their Plan of Care. Participates on the Behavior Team. Evaluate the effectiveness of psychotropic medication during Gradual Dose Reduction meetings.
  • Is aware of physical and mental condition changes for residents and refer them for appropriate treatment if needed.
  • Complete continuing education requirements.
  • Update care plans in MATRIX
  • Provides in-service training for staff on the emotional and social problems and needs of the aged and ill and on methods for fulfilling their needs.
  • Has strong communication skills and ethics. Maintain confidentiality and objectivity while dealing with residents and staff.
  • Comply with Facility Privacy Practices as governed by HIPAA requirements. Has access to full medical and admission records at Odd Fellow Home. Access to QAPI tools, complaint forms, QAPI reports, Committee meetings, wandering lists, and pre-admission information regardless of admission.
  • The Social Worker will be responsible to work with the Administrator on any grievances and lost items.
  • Must know where to find and interpret MSDS sheets. Follows facility safety practices.
  • Inform Administrator of current status of Social Services issues.
  • Initiate and update PASSRS as directed.
  • Complete and submit Medicaid Applications and complete renewals as needed.
  • Prepares and completes social service documentation in timely manner.
  • Other duties as assigned by management.

Qualifications and Education Requirements

  • Has a bachelor’s degree in social work, sociology, or psychology; meets the requirements for licensure in the state of Wisconsin; and have one year of social work experience in a healthcare setting.
  • OR, has a master’s degree in social work from a graduate school of social work accredited by the council on social work education.
  • Has a basic knowledge of computers or is able to learn the computer system currently set up in the facility.
  • Long Term Care Nursing Home background.

Job Type: Full-time

Pay: From $60,000.00 per year
