Take the attached pre-coded form to the Oberlin Police Department (85 S. Main St., Oberlin), which ensures results are sent to both ODE and the District. Payment is by check (payable to Oberlin Police Dept) or exact cash only. No appointment is required, but you may call 440-774-1061 to confirm the system is available. Results are typically processed within 7 days and sent directly from Ohio BCI/FBI to ODE.

The Coaches' Tool Chest certificate serves as proof that all requirements for the Pupil Activity Permit have been met. You must upload a copy of this certificate as verification of your Mental Health Training certification when completing your online application with the Office of Educator Licensure.
Effective October 3, 2023, Ohio law requires all athletic coaches to complete a student mental health training course approved by the Ohio Department of Mental Health and Addiction Services (OhioMHAS).
Please note that this is a single platformyou are trained once a year to meet all state compliance requirements. You no longer have to pursue and manage certifications from multiple platforms.
Pupil Activity Permit
If you do not currently hold a valid Ohio Pupil Activity Permit, you will need to apply through the Ohio Department of Education's online system (OH|ID). If you are advising per 3301-27-01, you will need to provide proof of your valid educator license.

All individuals employed or volunteering in co-curricular or extra-curricular positions must comply with applicable Board policies, including Policies 3120.08 and 4120.08, and the OOEA Negotiated Agreement, Article 7.07, which defines the extracurricular salary service index.
All supplemental and pupil activity assignments automatically terminate at the conclusion of the contractual activity each year, in accordance with Ohio Revised Code 3319.11.

- Successful completion of Bureau of Criminal Identification and Investigation (BCI) and Federal Bureau of Investigation (FBI) background checks, and enrollment in the State of Ohio Rapback System
- Completion of all required training through Coaches' Tool Chest (when applicable)
- Obtaining a valid Pupil Activity Permit from the Ohio Department of Education and Workforce in accordance with R.C. 3319.303 (when applicable)
