Oasis Therapy Services is seeking a Physical Therapist (PT) to provide one-on-one rehabilitation services in a skilled nursing setting in Prospect Heights, Illinois.

Responsibilities
- Evaluate mobility, strength, balance, gait, and fall risk.
- Develop and provide individualized treatment plans to improve safe mobility and independence.
- Monitor progress and adjust treatment plans as needed.
- Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Illinois Physical Therapist license.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
