Location: Oak Village Healthcare – Oaktown, IN
Schedule: PRN (As Needed)
Pay Rate: Up to $65/hr (5-10 hrs C-PRN Weekly)

Responsibilities:
Perform evaluations and develop individualized physical therapy treatment plans
Deliver evidence-based, patient-centered interventions
Document care accurately and update goals as needed
Collaborate with the interdisciplinary care team to support resident outcomes
Maintain compliance, quality, documentation, and ethical standards
Qualifications:
Licensed Physical Therapist (PT) in the state of Indiana
Current BLS certification
Strong clinical reasoning and communication skills
