Location: Oak Village Healthcare – Oaktown, IN

Oak Village Healthcare is seeking a skilled PRN Certified Occupational Therapy Assistant (COTA) to provide flexible occupational therapy services at our skilled nursing facility in Oaktown, IN. This position supports therapy programs designed to enhance residents’ independence and functioning under the supervision of an Occupational Therapist.
What You'll Do:
Deliver occupational therapy treatments following established standards, facility policies, and therapy department procedures.
Conduct resident screenings at regular intervals to assess the need for intervention or treatment, as permitted by state regulations and facility guidelines.
Assist the Occupational Therapist in developing and implementing individualized treatment plans.
Provide therapeutic interventions that promote independence, mobility, and participation in daily activities.
Document resident progress and communicate effectively with the interdisciplinary care team.
What You'll Bring:
Associate’s Degree in Occupational Therapy from an accredited program.
Current Occupational Therapy Assistant licensure in Indiana.
Must be 18 years or older.
Commitment to ongoing education and maintaining licensure and certification.
