As a key member of the Care Team, you’ll provide comprehensive primary care services under the supervision of an MD/DO, while playing a leadership role in patient care delivery.
- Perform patient histories, physical exams, and clinical assessments
- Diagnose conditions and develop individualized treatment plans
- Prescribe medications in accordance with NYS guidelines
- Perform minor procedures and manage acute and chronic conditions
- Review labs, imaging, and patient communications with timely follow-up
- Coordinate referrals and ongoing care needs
- Participate in outreach programs, quality initiatives, and team collaboration
- Active New York State PA license and DEA certification
- Board Certified (or eligible, with certification within 18 months)
- Strong clinical judgment and commitment to evidence-based care
- Commitment to teamwork, patient-centered care, and community health
