What are the responsibilities and job description for the Skilled Nursing Dietitian position at NUTRITION MANAGEMENT SERVICES?
GENERAL DUTIES:
Responsible for the nutritional care of all residents.
SPECIFIC RESPONSIBILITIES:
- Conducts the overall clinical operation of the facility by coordinating the nutrition care program for each resident which includes the assessment, planning, implementation, evaluation and delivery of nutrition services.
- Visits all new admissions within specified time frame for food preferences and special needs. Completes assessment forms and resident care plans within seven (7) days after admission. Completes MDS within specified time frame. Documents all pertinent nutritional information in the resident’s medical record in compliance with state and federal regulations.
- Represents the department by attending and participating in all resident-related meetings including Morning Meeting, Clinical Meeting, Resident Care Conferences, and Quality Improvement.
- Maintains a diet listing (Meal Tracker computerized system) on all residents, updating as necessary. Develops modified diet menu extensions based on the approved Diet Manual in conjunction with the Director and the Regional Dietitian. Assists in planning of special menus.
- Maintains Visitation Log and calendar system to coordinate resident visitation including MDS schedule, high risk residents, consults, etc.
- Available to Director and facility to provide inservice education to staff as needed.
- Assists the Director in developing and reviewing department policies and procedures.
- Evaluates the delivery of meals and nourishments to residents throughout the period of food service operation.
- Responsible for continued professional development. Keeps informed about current professional, managerial, and clinical developments.
- Maintains the Weight Monitor. Utilizes its program procedures to generate reports for dietitian and facility.
- Actively participate in the Weight and Wound Risk meetings per facility policy and prepares all reports as required on a timely basis. Participates in Wound Rounds, as appropriate.
- Maintains positive employee relations through effective communication.
- Conducts regular Test Tray and Tray Accuracy evaluations in conjunction with Director and Regional Dietitian.
- Supports sanitation program of department to meet health regulations.
- Remains flexible in assuming new or diverse responsibilities and exhibits a willingness to learn attitude in approaching new duties.
- Maintains resident confidentiality in accordance with HIPAA regulations.
- Keeps credentials current through continuing education and maintains A.D.A. membership or registration/licensure status.
- Supports and participates in company’s training program.
