We are hiring a PRN Home Health LVN for flexible, field-based visits in the Anahuac area. Scheduling is flexible, and visits are assigned as new patients are admitted, so the number of visits may vary from week to week. Many clinicians choose this role for the flexibility it offers alongside other professional commitments.
Responsibilities
- Provide skilled nursing care in patients’ homes
- Perform assessments, medication management and treatments per physician orders
- Complete documentation accurately and in a timely manner
- Communicate patient status changes with the care team
- Adhere to agency policies and state regulations
Experience & Training
- Home health experience preferred
- Clinicians without prior home health experience are required to complete agency training & One unpaid shadow visit prior to independently taking patients
- This position is not supervised in the field; clinicians must be comfortable working independently.
Qualifications
- Active LVN license in the state of Texas
- Reliable transportation
- Auto Insurance
- CPR Certification
- Strong time-management and communication skills
- Ability to work independently in the field
