Job Description:
The Home Health Intake Coordinator plays a critical role in ensuring the smooth and efficient onboarding of new patients into the agency’s home health services. This position is responsible for receiving referrals, verifying insurance, entering data into electronic systems, obtaining authorizations, and coordinating with internal staff to initiate timely care.
Key Responsibilities:
- Receive and process all incoming referrals from physicians, hospitals, and other sources.
- Enter patient and physician information accurately into electronic systems such as Kinnser/WellSky and Aida/Aidin.
- Utilize Aida/Aidin platforms to monitor, accept, and track hospital referrals efficiently.
- Obtain initial authorizations for home health services from insurance companies or other payers.
- Verify insurance coverage, eligibility, and benefits.
- Collaborate with the Patient Care Coordinator to ensure proper clinician staffing and timely start of care.
- Answer phone calls from referral sources and respond to inquiries regarding intake and services.
- Communicate with referral sources regarding the status and timing of patient admission and start of care.
- Ensure all required documentation is received and complete prior to initiating care.
- Maintain compliance with HIPAA, Medicare, Medicaid, and other regulatory standards.
- Work closely with clinical and administrative teams to streamline the intake process.
- Escalate urgent or complex intake issues to the Director of Nursing or Administrator as needed.
Qualifications:
- High School diploma or equivalent required; Associate’s degree or higher preferred.
- Prior experience in home health intake or related health care setting strongly preferred.
- Proficient in EMR systems (especially Kinnser/WellSky) and Microsoft Office Suite.
- Strong organizational skills and attention to detail.
- Excellent written and verbal communication skills.
- Ability to multitask and prioritize in a fast-paced environment.
- Ideal candidate is self-directed, able to work with minimal supervision, and demonstrates critical thinking and problem-solving skills.
- Knowledge of Medicare/Medicaid guidelines and home health regulations is a plus.
