Role Description
This is a full-time, on-site role for a Care Coordinator located in Wilmington, DE. The Care Coordinator will be responsible for managing and coordinating care for clients and providing support to both clients and their families. Daily tasks include maintaining communication with clients, families, and healthcare providers, and ensuring the highest level of customer service in home care settings.
