Responsibilities
- Complete comprehensive occupational therapy evaluations and reassessments
- Develop individualized plans of care based on each patient’s functional needs and personal goals
- Provide skilled treatment focused on activities of daily living, upper-extremity function, strength, coordination, cognition, and safety
- Assess home environments and recommend modifications, adaptive equipment, and assistive devices
- Provide training related to bathing, dressing, toileting, meal preparation, and other daily activities
- Educate patients, families, and caregivers on safety strategies and therapeutic exercises
- Coordinate care with nursing, physical therapy, physicians, and other providers
- Participate in interdisciplinary communication and care planning
- Complete accurate and timely clinical documentation
- Communicate changes in condition and safety concerns promptly
- Current Connecticut Occupational Therapist license
- Graduate of an accredited occupational therapy program
- Valid driver’s license and reliable transportation
- Strong communication, organization, and documentation skills
- Ability to work independently while collaborating with the clinical team
- A positive attitude and commitment to patient-centered care
