Are you passionate about supporting individuals and families as they work toward stability, wellness, and long-term success? NCSS is seeking a compassionate and motivated Reach Up Wellness Coach to join our Adult Home and Community Based Services team. In this role, you’ll partner closely with Reach Up participants and families, helping them navigate challenges, build confidence, and work toward meaningful goals.
As a Wellness Coach, you’ll develop trusting relationships with program participants, providing case management support across a wide range of life areas—such as childcare, budgeting, healthy relationships, skill-building, housing supports, parenting, and overall life balance. You’ll play a key role in assessing needs, connecting individuals to the right resources, collaborating with Reach Up Case Managers/Career Coaches, and helping participants reach their highest potential through structured plans, coaching, and encouragement.
This position blends direct support, care coordination, and teamwork. You’ll conduct home or office visits, participate actively in treatment and service planning, and serve as an important link between participants and internal/external service providers. You’ll also contribute to the overall Reach Up team through case consultations, shared planning, and regular onsite presence at the local Economic Services Division office.
Key Responsibilities
- Support a caseload of Reach Up participants through case management, goal planning, and routine progress evaluation.
- Provide screenings, coordinate services, and offer education around mental health and substance-use topics.
- Collaborate with Reach Up Case Managers/Career Coaches and participate in team meetings, treatment planning, and weekly consultations.
- Coordinate access to community resources, clinical services, and supports while ensuring compliance with agency and state documentation standards.

Bachelor’s Degree; or equivalent combination of related education, training and experience in health coaching and patient education; knowledge of healthcare systems, chronic disease, healthy lifestyles, a plus.
