POSITION DESCRIPTION:
Northwest Resources II, Inc. provides a residential level of care (3.5) for up to 16 male patients, with a diagnosis of substance use disorder. This position requires a customer service focus as this is the first contact with the program for both patients and referents. The Intake Coordinator is responsible for answering calls and questions related to the program. Ensure all appropriate documentation is collected to ensure that patient placement criteria are met. This position schedules and admits patients.
RESPONSIBILITIES:
- Responsible for maintaining a census of 16 male patients appropriate for 3.5 level of care program.
- Ensure all appropriate documentation is collected prior to admission and completed at the time of admission.
- Conducts thorough search of belongings and separate contraband and medications from belongings.
- Verify insurance coverage and eligibility on an individual basis prior to admission and at the beginning of each month.
- Maintain relationships and communication with both referents and the treatment team.
- Maintain a wait list, to include documenting on the Targets First Contact Form, and maintain weekly contact with all individuals on the waitlist.
- Prepare new patient files, ensuring all admissions forms are complete and provided to the treatment team in a timely manner.
- Participate in weekly clinal staffing to collect discharge dates and inform staff of scheduled admissions.
- Interact and intervene with patients constructively and consistently, in line with the treatment philosophy, policies and procedures as well as the individual treatment plan.
- Alert colleagues and administrators in the event of emergencies, unmanageable inappropriate behavior, threats to safety, health and/or security.
- Complete incident reports as indicated and directed by policy and procedure.
- Maintain strict confidentiality regarding patient records and communication.
- Maintain professional boundaries as outlined by NWRII.
- Scan and attach documents to patient charts.
- Complete and enter required Washington state admission and discharge data.
QUALIFICATIONS:
- High School Diploma or GED.
- Ability to communicate and document events clearly.
- Ability to effectively present information and respond to questions from groups, clients and colleagues.
- Valid WA State Driver License.
- 1-year experience using Microsoft Office Suite.
