Northwest Home Health & Rehab is an independent, family owned, Medicare-certified home health agency that has been in business since 2004. We are known by our referral sources as providing the highest quality of care. We employ the best in the business and are looking to hire experienced registered nurses (RN’s) to join our team! We are seeking experienced field RN’s to join our healthcare team as a part-time nurse. You’ll enjoy the flexibility and autonomy of creating your own weekly schedule along with highly competitive pay as well as a generous signing bonus.

Service Area: Parts of Cook and Lake County

Hours: Days – Monday through Friday

As a home health care RN with Northwest Home Health and Rehab, your visits are tailored to your geographical location and schedule.

Home Health RN Responsibilities

· Function as a case manager in charge of caseload

· Complete initial Start of Care OASIS visit and regularly re-evaluate the patient’s nursing needs

· Collaborate with physicians and the health care team in determining the care plan and needs of the patient

· Educate patients, caregivers and family members

· Work closely and collaboratively with all members of the health care team

· Inform the Clinical Manager of patient progress and clinical concerns

· Record patient medical information in our electronic medical record system, Axxess

· Inform the physician of changes in the patient’s condition or treatment plans

Not required to come in to the office.

Home Health RN Qualifications

· Registered Nurse licensed in the state of Illinois

· Current driver’s license, auto insurance and CPR

· Nursing experience in home health

· Effective written and verbal communication skills

· Strong desire to help others

· Wound care experience

Pay: $65.00 - $110.00 per hour
