What are the responsibilities and job description for the Substance Use Disorder Counselor position at Northshore Home Health Care?
We are seeking a Substance Use Disorder Counselor to provide individual and group counseling, conduct assessments, screenings, and treatment planning, and deliver case management services. The role involves maintaining accurate clinical documentation, supporting clients in recovery and stability, and collaborating with multidisciplinary teams and community partners.
Qualifications include a valid Wisconsin SAC-IT, SAC, or CSAC credential, an associate's degree in human services or related fields (bachelor's preferred), and knowledge of addiction, recovery, and case management principles. Experience with justice-involved populations is a plus. A valid driver’s license and reliable transportation are required.
