- Deliver comprehensive end-of-life and palliative care, including pain management and supportive measures.
- Assess patient conditions, monitor symptoms, and communicate effectively with patients, families, and interdisciplinary team members.
- Document care accurately within electronic medical records in a timely manner.
- Graduation from an approved nursing program and current licensure in the state of practice.
- CPR certification and valid driver’s license.
- Preferred hospice or long-term care experience, with knowledge of death, dying, and bereavement issues.
- Strong assessment, observation, communication, and computer skills.
- Self-directed, flexible, and capable of working independently and collaboratively within an interdisciplinary team.
