Position: Primary Care Nurse (RN or LPN) in a hybrid remote setting, working closely with primary care providers to deliver patient-centered, efficient, and equitable care.
Responsibilities: Partner with patients, providers, and the clinical team to drive quality health outcomes, ensuring safe and effective care in accordance with mission, values, and standards. After initial in-person training, perform duties remotely 2-3 days weekly.
Qualifications: Diploma from an accredited nursing program (RN or LPN), active Oklahoma nursing licensure, and preferred BSN. Minimum six months of nursing experience; primary care experience is a plus but not required.
