- Providing individualized, one-on-one nursing care in patients' homes to promote safety, recovery, and independence.
- Conducting comprehensive in-home assessments to develop patient-centered care plans.
- Performing skilled interventions such as wound care, infusions, medication management, and post-operative support.
- Educating patients and caregivers to manage symptoms, medications, and daily care confidently.
- Monitoring patient progress, identifying changes, and communicating with the care team to adjust plans and prevent hospitalizations.
- Ensuring care continuity through coordination across providers and timely, accurate documentation.
- Active RN license in the practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
