We are seeking a compassionate Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide direct patient care in the East Valley area, supporting home health services.

- Collaborating with healthcare team members to ensure effective patient care according to treatment plans.
- Conducting patient visits following physician instructions and regulations.
- Observing and documenting patients' physical and mental conditions.
- Reporting changes in patient status promptly.
- Providing patient and caregiver education on care plans and health management.
- Participating in case conferences and maintaining accurate documentation.
- Graduate of an accredited practical nursing program and licensed in the state.
- Current CPR/BLS certification.
- Minimum of one year home health experience.
- Strong clinical, communication, organizational, and interpersonal skills.
- Valid driver's license and auto insurance as required.
- Experience with electronic health records (EHR) systems is preferred.
