POSITION: Wellness and Engagement Coordinator
LOCATION: Grand Rapids, MN
REPORTS DIRECTLY TO: Housing Manager and Registered RN
JOB REQUIREMENTS:
- Must possess the skills, boundaries, and work ethic required for the position
- Must possess a valid driver’s license
- Ability to multi-task, possess leadership skills, articulate and document clearly, demonstrate flexibility, maintain confidentiality, balance a variety of tasks, and prioritize responsibilities
RESPONSIBILITIES FOR TENANT SERVICE COORDINATOR POSITION:
- Monitor medications, offer reminders, and prompts while clearly documenting services provided
- Coordinate with Pharmacy, Medication Manager, Primary Doctor, and Nurse as it relates to instructions, refills, orders, questions, or changes needed
- Keep record of refills, orders, and deliveries
- Assist tenants as needed through providing support and services that are person centered
- Monitor site and report any maintenance related issues to the property managers
- Maintain offices and front desk space as needed
- Assist with activities, events, bingo, and offer consistent tenant engagement opportunities
- Coordinate with team, property managers, outside agencies, and providers as needed/requested for collaboration and referrals
- Attend staff meetings/trainings as requested/scheduled
- Other duties as assigned
HOURS:
- Part Time Hourly
- Staff may occasionally be required to come in early or work late as needed to accommodate the site
SALARY: Based on education, experience, awareness of job-related duties, and completion of training required for the position with a starting range of $16.00 - $17.00 per hour
