Position Summary
The Patient Care Coordinator serves as the primary point of contact for patients, caregivers, physicians, referral sources, and insurance companies for simple DME/HME.
This is an entry-level healthcare position focused primarily on customer service and coordinating basic rehabilitation equipment and standard durable medical equipment (DME). You will guide patients through the equipment process from referral to delivery while ensuring every interaction reflects our commitment to outstanding service.

What You'll Do
- Serve as the first point of contact for patients, caregivers, physicians, hospitals, and referral sources.
- Answer incoming phone calls promptly, professionally, and courteously.
- Greet patients and visitors while creating a welcoming and positive experience.
- Coordinate referrals for basic rehabilitation equipment and standard durable medical equipment.
- Schedule patient appointments, equipment deliveries, pickups, and follow-up visits.
- Assist patients in understanding the equipment ordering process and expected timelines.
- Obtain physician documentation, prescriptions, and supporting medical records with support from our processing team.
- Communicate with physician offices, therapists, referral coordinators, insurance companies, and patients regarding referral status.
- Enter and maintain accurate patient information within company software.
- Coordinate with warehouse staff and delivery technicians to ensure timely equipment delivery.
- Follow up with patients regarding equipment status and customer satisfaction.
- Resolve customer concerns with professionalism and empathy.
- Maintain accurate documentation while protecting patient confidentiality in accordance with HIPAA regulations.
- Support front office operations and assist other departments as needed.
- Contribute to a positive, collaborative team environment focused on exceptional patient care.

Required Qualifications
- High school diploma or GED.
- At least one (1) year of customer service experience.
- Excellent verbal and written communication skills.
- Strong interpersonal and relationship-building skills.
- Outstanding organizational skills with strong attention to detail.
- Ability to prioritize multiple tasks in a fast-paced environment.
- Professional telephone etiquette.
- Basic computer proficiency, including Microsoft Office.
- Ability to learn new software and healthcare processes quickly.

Preferred Qualifications
- 1–3 years of Durable Medical Equipment (DME) experience.
- Customer service experience in a healthcare setting.
- Experience in retail, hospitality, banking, medical reception, or another high-volume customer service environment.
- Experience with insurance verification.
- Familiarity with Medicare and Medicaid.
- Experience using Brightree or other healthcare management software.
- Knowledge of medical terminology.
