Summary of Responsibilities
This position supports Physician Services and/or I/DD Clinical Support Services. Primary responsibilities include completing nursing assessments, monitoring individuals’ health care needs, and administering or overseeing injections. Services may be provided in outpatient clinics, community settings, individuals’ homes, or Adult Foster Care (AFC) environments.
The role includes completing Annual Assessments, Pre‑Plans, Plans of Service, POSRs, Medication Reviews, Nursing Assessments, and Quarterly Nursing Reviews as needed. Additional duties involve developing integrated health care programs, overseeing laboratory work and physician‑ordered treatments, consulting with providers and staff, training personnel on program procedures, monitoring treatment progress, and completing case closures.
The position is also responsible for addressing daily medication calls, consulting with physicians, physician assistants, and nurse practitioners, and managing discharges from physician services. As needed, the nurse may meet with pharmaceutical representatives to record medication samples and assist medical staff in distributing them. Participation in crisis‑intervention backup services may be required.
Behavioral Health Home (BHH) provides comprehensive care management and coordination for individuals with serious mental illness or serious emotional disturbance who are enrolled in Medicaid, the Healthy Michigan Plan, or MIChild.
BHH emphasizes integrated, person‑centered care that addresses complex co‑occurring physical and behavioral health conditions.
Qualifications
Education:
- Registered Nurse degree from an accredited institution with coursework in mental health.
Experience:
- At least one year of relevant mental health experience.
- Strong verbal and written communication skills required.
- Must hold State of Michigan RN licensure. Functional computer literacy required within six months of hire.
- Lived experience with mental illness, intellectual/developmental disabilities, or substance use disorders is valued.
Essential Job Functions
- Demonstrate knowledge of universal precautions, professional standards of practice, confidentiality, recipient rights, and emergency protocols.
- Conduct accurate assessments and diagnoses in accordance with DSM‑5 (or current edition).
- Complete comprehensive health care assessments and develop person‑centered health care plans and guidelines.
- Maintain accurate case documentation following agency procedures.
- Apply knowledge of medication dispensing procedures in AFC or personal home settings.
- Provide education, crisis intervention, and individual or family support as needed.
- Collaborate with staff in evaluating concerns and identifying needs of individuals served.
- Comply with guidelines established by PIHP, MDHHS, Medicaid, and other regulatory agencies.
- Maintain effective communication and positive working relationships with clinical staff, case managers, external agencies, primary care providers, and support staff.
- Demonstrate functional computer literacy in all required documentation.
- Maintain required licensure or registration at the highest applicable level.
- Participate in training related to co‑occurring disorders.
- Provide in‑service training and consultation to staff, individuals served, families, and agencies as needed.
- Assist Physician Services with physician action requests, triage calls, vital signs, initial lab orders, and tele‑psychiatry appointments, as assigned.
Non‑Essential Job Functions
- Perform other duties as assigned.
- Participate in continuous quality improvement activities as requested.
- Travel to agency offices and sites as needed.
- Provide personal transportation for agency‑related duties when required.
- Participate in 24‑hour emergency services coverage as needed.
- Complete 24 hours of continuing education annually per Policy 5135.
