Requirements:
- High school diploma, Bachelor's degree highly preferred.
- Knowledge of and experience with seriously and persistently mental ill adults preferred.
Responsibilities:
Team Lead
- Over-see medication delivery duties by CIS (in conjunction with the CRT Nurse)
- Managing daily schedule for CRT staff, including last minute schedule changes or triaging crisis
- Maintain weekday and weekend point schedules
- Assist in facilitating weekly clinical meetings
Service Coordination
- Promote community integration of individuals and assist/encourage natural supports necessary for each individual to participate in the community.
- Develop & implement IPC in accordance with client needs and Treatment Team recommendations.
- Provide Special Rehab/Case Management services including training in basic living/home management skills, social skills, supportive counseling, service coordination & treatment planning.
