What are the responsibilities and job description for the Therapeutic Case Manager - Turning Points School position at Northeast Kingdom Human Services?
Monday-Friday, 7:30am - 4:00pm (30 minute lunch break included)
Requirements:
- Master's in human service field preferred; will consider Bachelor's with relevant experience.
- Experience with emotionally/behaviorally challenged children and youth highly desirable.
Responsibilities:
- Meet with or have telephone contact with children and parents for evaluation, individual and family supportive interventions, crisis management and communication of significant information.
- Coordinate clinical treatment team meetings with students, parents and treatment team providers.
- Responsible for ongoing development and implementation of transition plans with referring agencies and/or school systems, and potential post program placements.
