What are the responsibilities and job description for the Social Worker (Hospice) - Newberry to Sault Ste. Marie position at North Woods Home Care & Hospice?
North Woods Home Care and Hospice is seeking a Hospice Social Worker (MSW/LLMSW) to provide medical social services to clients in the Sault Ste. Marie and Newberry areas. Employee would prepare social histories and provide social casework to patients and families according to agency standards and policies.
Job Functions May Include: Assess patient’s/family’s psycho-social status. Provide counseling or intervention to patients/families according to plan of care. Develop and implement a plan of treatment for patients/families in consultation with the physician. Assist in understanding the social and emotional factors related to health problems. Establish goals in collaboration with patient/families. Prepare clinical/progress notes for each visit. Assist patients/families in utilizing community resources. Assist in evaluation of patient’s insurance coverage and assess their ability to meet financial obligations.
Requirements: Masters Level Social Work License may be LLMSW. Valid driver’s license with good driving record. Must communicate well with patients/families and be trustworthy. Prior experience is a plus. Fingerprinting and background check will be conducted.
