Summary: 

The OBAT Coordinator is a vital member of the NOHN Nursing Services team, providing comprehensive care and support to patients receiving Office Based Addiction Treatment (OBAT) services. You will work closely with patients, medical providers, and the care team to ensure coordinated, patient-centered care that promotes recovery and overall well-being. This role requires a compassionate, organized, and resourceful individual with a strong commitment to harm reduction and evidence-based practices.  

Essential Duties and Responsibilities: 

Direct Patient Care: 

  • Engage and develop rapport with patients, building trust and reducing stigma.  
  • Conduct screenings, schedule appointments, and assist patients with insurance coverage issues.  
  • Coordinate and facilitate patient-centered care for individuals receiving OBAT services.  
  • Provide education and support to patients regarding OBAT services, harm reduction, and recovery resources.  
  • Identify and address barriers to care proactively, while maintaining patients' dignity and autonomy.  
  • Advocate for patients' needs while maintaining clear boundaries and promoting accountability.  
  • Maintain accurate and complete patient records in the electronic health record system (EHR). 

Care Coordination and Collaboration: 

  • Work collaboratively with the care team (medical providers, nurses, counselors) to ensure coordinated care.  
  • Communicate effectively with providers and the multi-disciplinary health care team.  
  • Facilitate patient registries, care delivery appointments, and community outreach activities.  
  • Work in partnership with external addiction facilities, behavioral health, and primary care partners.  
  • Participate in case conferences and team meetings to discuss patient progress and challenges. 

Program Support and Administration: 

  • Provide direct non-clinical program support and administrative services.  
  • Conduct community outreach with prospective referral sources.  
  • Complete all required OBAT and new patient paperwork.  
  • Assure the program follows administrative protocols.  
  • Assist with grant tracking, data entry, and reporting.  
  • Document patient information accurately, concisely, and in a timely manner.  
  • Demonstrate competence in information management in both written and electronic documentation systems.  
  • Help assure compliance with Federal, State, and program guidelines.  
  • Safeguard confidential information.  
  • Uphold and support the organization's mission statement.  
Qualifications:

Education: Completion of post-secondary education is preferred but not required.  

Experience:  

  • A minimum of two (2) years of program management in a healthcare setting preferred.  
  • A minimum of two (2) years working with substance use disorder programs or related field preferred.  

Skills: 

  • Exceptional patient skills, demonstrating empathy, compassion, and cultural sensitivity.  
  • Excellent communication and interpersonal skills, both written and verbal.  
  • Strong organizational and time management skills, with the ability to prioritize tasks effectively.  
  • Proficiency in EHR systems and Microsoft Office Suite.  
  • Ability to work independently and collaboratively as part of a team.  
  • Knowledge of community resources and harm reduction strategies.  

Language: Bilingual in Spanish preferred.  

Physical Demands: May require prolonged sitting, standing, and some lifting of light objects (up to 20 lbs).  

Work Environment: Fast-paced community health center setting, potential exposure to infectious diseases, interaction with diverse patient populations.
