What are the responsibilities and job description for the Senior Case Manager position at North Central Texas Council of Governments?
The purpose of this position is to serve as Person-Centered Counselor in support of the Veterans Administration's Veteran Directed Care (VDC) Program. As such, the employee will have responsibilities that include:
A Bachelor's degree in social work or related human services field is preferred and 3-5 years related work experience. At least two years' case management experience is desired, preferably working with older adults, persons with disabilities, and/or veterans. In addition, the position requires strong interpersonal and communication skills, ability to work independently and manage a caseload, and proficiency in documentation (using both Word and Excel) and electronic case management systems.
The starting salary is $57,789 per annum ($27.78/hour).

- Conduct comprehensive person-centered assessments to identify veterans’ strengths, needs, and preferences.
- Collaborate with veterans, their families, and caregivers to develop individualized service plans.
- Create spending plans for approval by the Veterans Administration.
- Serve as a liaison between the veteran and the fiscal management service agency.
- Educate veterans on self-direction, budgeting, and hiring/managing personal care attendants.
- Provide ongoing counseling and support to ensure veterans are successfully managing their services.
- Monitor service delivery and adjust plans as needed to ensure quality and satisfaction.
- Maintain accurate and timely documentation in accordance with program and funding requirements.
- Coordinate with VA Medical Centers, fiscal intermediaries, and community-based organizations.
- Advocate for veterans’ rights and access to services that promote autonomy and dignity.
