Hmong Community Outreach Specialist

Hours: 20 to 30 hours per month

Schedule: Varied (Tuesday evenings plus other flex hours as needed)

Location: Hybrid (Remote & On-Site) -- Kansas City, Kansas

Job Summary: The Community Outreach Specialist supports Fountain of Life initiatives to the Hmong-speaking community. The Community Outreach Specialist will serve as a health navigator/community health worker/social worker to help connect individuals and families in need to health, wellness, aging, social, housing and legal services that they need. When individuals whose primary language is Hmong reach out to Fountain of Life, the Community Outreach Specialist will be their first point of contact.

Job Responsibilities: The Community Outreach Specialist key responsibilities center on two goals:

• Connect Hmong-speaking individuals and families, especially those who are uninsured or underinsured, to health, aging, government and community resources including medical, dental, behavioral health care resources that they need to lead healthy lives
• Promote Fountain of Life social, recreational and mental health programs to Hmong-speaking older adults
  
  

To accomplish these two goals, the Community Outreach Specialist will:

• Represent Fountain of Life to Hmong-speaking community of Greater Kansas City
• Help develop and maintain relationships between Fountain of Life and Hmong-serving community organizations, churches and groups
• Conduct intakes on individuals and families to determine their health, wellness, aging, social, housing and legal needs
• Educate and connect individuals and families to community resources and services to meet their needs
• Make follow up phone calls and serve as point of contact for Hmong-speaking individuals and families reached through health outreach events to provide continued care coordination and address any barriers they are experiencing in accessing community resources and services
• Maintain relationships with individuals and families and encourage participation in Fountain of Life’s programs and activities at its senior community center located in Kansas City, Kansas
• Translate program flyers to Hmong and help distribute Fountain of Life’s program information to the community
• Serve as Hmong interpreter at Fountain of Life’s programs and activities as needed
• Track program participant information according to Fountain of Life’s program participation tracking procedures
  
  

Required Qualifications

• A sincere heart to build relationships among the different Asian ethnic communities
• Ability to use computer applications including Microsoft, Google Suite, and email
• Bilingual in Hmong and English
• Legal resident and eligible to work in the United States
• Reliable transportation / access to fully-insured vehicle to use for job-related duties

Compensation: $25 per hour for 8-month period (May 2026 to December 31, 2026).
