What are the responsibilities and job description for the CASE WORKER position at Nobles Group Homes?
**OFFICE HOURS : MONDAY- FRIDAY 8AM - 5PM**
DUTIES AND RESPONSIBILITIES
- Conduct assessments.
- Assist in developing service plans.
- Coordinate services.
- Provide advocacy and support.
- Work closely with Individuals, their families and QIDPs, to identify needs.
- Help Individuals access services, including counseling, or financial assistance.
- Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses as applicable.
- Advise family caregivers.
- Provide Individuals with information and counseling.
- Make referrals for other services.
- Monitor visits between at-risk or fragile children and family members.
- Motivate individuals to participate in rehabilitation programs.
- Report abuse.
MINIMUM QUALIFICATIONS
Education
Bachelor's degree in a behavioral science discipline such as Social Work, Sociology or Psychology; or a bachelor’s degree in science or arts.
Skill and Ability to:
Accurately type and learn to efficiently use various software applications; communicate effectively in oral and written form; follow instructions; organize and prioritize work; perform basic research and report results; read and apply rules, regulations, and procedures; proofread material and make necessary corrections; learn and follow standard office procedures; learn appropriate filing and recordkeeping systems; select appropriate business formats; work cooperatively and effectively with students, parents, administrators, staff, and the general public.
