GENERAL DESCRIPTION:DCF Credentialed Case Manager is responsible for providing the Noank Community Support Services contracted DCF services of
· CHAP-Community Housing Assistance Program
· CHEER- Community Housing Employment Enrichment Resources
· SS-Support Staff
· SV-Supervised Visitation
· TC-Temporary Care
· TSS-Therapeutic Support Staff
MAJOR DUTIES AND RESPONSIBILITIES:
· Assist with securing housing and employment within the DCF established guidelines for the region
· Implement comprehensive treatment plans for successful young adult life skills progress
· Provide site-based services to facilitate contact between children and family members
· Collaborate with DCF and other providers towards the implementation of the child/youth treatment plan
· Able to follow and execute multiple directives given by supervisor in an effective and productive manner
· Monitor clients’ attendance and performance at educational and/or employment site(s).
· Provide ongoing support, consultation, and problem-solving assistance to client(s) a minimum of 10 hours a week for the first month and approximately 5 hours per week thereafter for a period of time determined by DCF or until clients reaches the age of 23.
· Complete assigned progress, collateral, and monthly progress reports by determined due dates.
· Attend All staff meetings, trainings, and supervisions scheduled by the supervisor.
· Be available to assigned clients on an emergency or urgency basis. You are expected to carry a cellular/mobile phone to fulfill this responsibility. NCSS will provide a second line for your mobile phone via a phone app.
· Mentor young clients with behavioral concerns and implement a life skills case management treatment plan
· Assess, engage, and/or initiate child and parent interaction during supervised visitation hours deemed by DCF, and complete all report(s) within 24 hours to supervisor.

QUALIFICATIONS: CHAP/CHEER, SS, TC, TSS: A degree in Human Services or related field and a minimum of three years of experience, an Associate’s with 5 years of experience or seven (7) years of experience working with at-risk youth
SV: A degree in Human Services or related field required. Familiarity with case management procedures, techniques, and trauma-informed care. Ability to effectively work with DCF Social Workers and other providers. Must have a valid driver’s license, the availability of a safe and roadworthy vehicle and provide the Agency with proof of suitable automobile insurance.
The Case Manager is required to be able to operate a personal computer with proficiency in the use of Microsoft 365, web-based platforms, and Adobe PDF. The Case Manager must pass a national background check conducted by fingerprints, a state police background check, a CT DCF protective service background check, a motor vehicle check, and provide proof of physical and negative PPD results. If the applicant has resided in another state in the last 5 years background checks must be conducted and passed in those states as well.
